1. Acceptance of Terms

These Terms & Conditions ("Terms") constitute a legally binding agreement between you ("User," "Patient," or "you") and Health Balance Source LLC ("Company," "we," "us," or "our"), a healthcare services provider organized under the laws of the State of Arizona, USA. By accessing our website (healthbalancesource.com), booking services, or receiving care, you acknowledge that you have read, understood, and agreed to be bound by these Terms.

2. Eligibility

You must be at least 18 years old to use our services. Minors may receive care only through a parent or legal guardian who agrees to these Terms on their behalf. You represent that you have the legal capacity to enter binding agreements and that all information you provide is accurate and complete.

3. Services Provided

3.1 Scope

Health Balance Source provides primary healthcare, senior care, cardiology, skincare treatments, hair transplantation, dental care, telemedicine consultations, wellness memberships, and related health services across the United States.

3.2 No Guarantees

Medicine is an inexact science. While we employ the highest standards of care, we cannot guarantee specific outcomes, cures, or results. All treatments carry inherent risks that will be disclosed during informed consent.

4. Account Registration

To access certain services, you must create a patient account. You agree to:

• Provide truthful, accurate, and current information
• Maintain the security and confidentiality of your login credentials
• Immediately report unauthorized access or security breaches
• Accept responsibility for all activities under your account

5. Appointments & Cancellations

• Appointments must be booked through our website, app, or phone
• Cancellations require 24 hours' notice to avoid a $50 late-cancellation fee
• No-shows may incur a $75 fee and result in service suspension
• We reserve the right to reschedule or cancel due to emergencies

6. Payment Terms

6.1 Fees & Billing

Service fees are disclosed prior to booking. Payment is due at the time of service unless insurance is verified. Membership plans auto-renew monthly until cancelled.

6.2 Insurance

We accept most major US insurers. You are responsible for co-pays, deductibles, non-covered services, and any balance not paid by your insurer.

6.3 Refunds

Refunds are processed within 7–10 business days for eligible services. Unused membership fees may be refunded on a prorated basis. Procedure deposits are non-refundable after scheduling.

Information on our website is for educational purposes only and does not constitute medical advice, diagnosis, or treatment. Always consult a qualified healthcare provider before making medical decisions. In a medical emergency, call 911 immediately.

8. Patient Responsibilities

You agree to:

• Provide complete and accurate medical history, including medications and allergies
• Follow prescribed treatment plans and attend follow-up appointments
• Report any adverse reactions or concerns promptly
• Respect staff, other patients, and our facilities
• Pay for services rendered per the agreed terms

9. Intellectual Property

All content on our website—including text, images, logos, videos, designs, and software—is owned by Health Balance Source or licensed to us and protected by US and international copyright, trademark, and other intellectual property laws. You may not copy, distribute, modify, or create derivative works without prior written consent.

10. Prohibited Conduct

You agree not to:

• Use our services for unlawful or fraudulent purposes
• Submit false information or impersonate others
• Reverse-engineer, scrape, or hack our systems
• Harass, threaten, or abuse staff or other patients
• Distribute malware or engage in disruptive activities

11. Limitation of Liability

To the maximum extent permitted by law, Health Balance Source shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising from your use of our services, including but not limited to lost profits, data loss, or emotional distress. Our aggregate liability for any claim shall not exceed the amount you paid us in the 12 months preceding the claim.

12. Indemnification

You agree to indemnify, defend, and hold harmless Health Balance Source, its officers, employees, and affiliates from any claims, damages, losses, or expenses (including legal fees) arising from your breach of these Terms, misuse of services, or violation of any law or third-party right.

13. Termination

We may suspend or terminate your account or access to services at any time, with or without cause, and with or without notice. Upon termination, your right to use our services ceases immediately. Sections that by their nature should survive termination will remain in effect.

14. Dispute Resolution

14.1 Governing Law

These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law principles.

14.2 Arbitration

Any dispute shall be resolved through binding arbitration administered by the American Arbitration Association in Tucson, Arizona. You waive your right to a jury trial and to participate in class-action lawsuits.

14.3 Informal Resolution

Before filing arbitration, you agree to contact us to attempt informal resolution for at least 60 days.

15. Severability

If any provision of these Terms is held invalid, illegal, or unenforceable, the remaining provisions remain in full force and effect.

16. Modifications

We reserve the right to update these Terms at any time. Material changes will be posted on this page with a new effective date. Continued use of our services after changes constitutes acceptance of the revised Terms.
